At Conduit Health Partners, our success is driven by dedicated associates who support their teams and ensure seamless patient care. Meet Hannah Hardy BSN, RN, Supervisor Line Lead at Conduit, whose leadership and passion for empowering others make a difference every day.

A Leader in Patient Transfers

Since joining Conduit in May 2021, Hannah has played a key role in overseeing a team of nurses, managing schedules, and ensuring top-notch performance within the Transfer Center. With a career background involving a role at Bon Secours Mercy Health since 2016, she brings a wealth of experience and a deep commitment to excellence.

Inspired by the strong leadership within the Transfer Center, Hannah was drawn to her current role by a team that fosters empowerment, clear communication and recognition. “Our leaders lead by example and have improved processes to help us meet our performance targets to succeed as a Transfer Center,” Hannah shares. “Seeing how well they lead our team inspired me to want to be a part of that.”

Making an Impact

For Hannah, the most rewarding part of her job is uplifting her team. “I love making my team members feel valued and encouraging them to excel,” she says. Whether through recognition or well-deserved shoutouts, she takes pride in recognizing their hard work.

While she may not be at the bedside, she knows her work is vital to patient care. “Our Transfer Center is critical in ensuring safe and timely patient transfers,” she explains. Communicating with bedside staff and coordinating urgent cases fuels her passion for nursing and helping others.

Looking Ahead

As Conduit continues to grow, Hannah is excited for what’s ahead. She looks forward to new challenges, professional development and contributing to a thriving organization.

“I enjoy being a part of such a strong team and look forward to continuing to grow and challenge myself along the way,” Hannah shares.

Beyond Work

Outside of work, Hannah enjoys spending time with her husband and three children—often found on a ball field or basketball court. She’s also active in her church and small group, and when time allows, she loves skiing out west.

Her motivation? Seeing a plan come to life. “I enjoy being challenged and learning new skills,” she says. And with her drive and dedication, there’s no doubt she’ll continue making an impact at Conduit Health Partners.
