How Nurse-First Triage and Transfer Centers Are Reshaping Health Care—Insights from Conduit’s COO

In a recent conversation with Matthew Holt of The Health Care Blog, Dominique Wells, MSN, RN, chief operating officer at Conduit Health Partners, shared insights into how nursing is evolving, and how Conduit Health Partners is at the forefront of this transformation.

Conduit Health Partners supports overburdened nursing teams with high-impact services including patient transfer operations and  nurse-first triage. As Dominique explained, these services don’t replace bedside care, but they strengthen it by taking pressure off health system staff and filling in critical operational gaps.

The Evolving Role of Nurses

The conversation turned to a timely topic: how nursing has changed since the COVID-19 pandemic. Dominique shared the demands placed on clinical teams have only grown, but resources haven’t always kept pace. Nurses are being asked to do more with less, while also navigating new technologies and patient expectations.

Conduit Health Partners’ clients have seen how outsourcing key services, like coordinating patient transfers or managing after-hours triage, gives nurses the space they need to focus on what matters most: patient care.

Transfer Center in Action

Dominique walked Matthew through a brief demo of Conduit Health Partners’ patient transfer center, one of their fastest-growing areas of support. The outsourced model streamlines how patients move across the continuum of care, reducing delays and ultimately improving patient outcomes.

With a clinically led team handling the logistics, bedside nurses are no longer stuck on hold trying to find an open bed or coordinate transportation.

The team shared a simulation of how the transfer center process works in real time.

AI and the Future of Nursing

No conversation about the future of health care would be complete without addressing artificial intelligence. Dominique and Matthew explored how AI might support, but never replace, nursing care. Dominique shared that at Conduit Health Partners, we view AI as a tool to enhance clinical decision-making and streamline workflows, especially in triage and communication services.

But Dominique was clear: “Technology must be designed with nurses, for nurses. Their expertise is central to every innovation we deploy.”

Supporting the Most Vital Role in Health Care

Nurses remain the backbone of health care, but they shouldn’t be carrying the weight of the system alone. Conduit Health Partners is working to redefine how support services can enable nursing teams to do more, with less stress, fewer administrative tasks and stronger outcomes.

As Matthew Holt reflected, it’s clear that new services like those we offer are being built not just to respond to today’s challenges, but to shape a smarter, more sustainable future for health care.
